FDA Appeal - January 2006 Parents: Please follow these instructions: First, in the chart below check the column on the left for the Region we have assigned. These Regions have been chosen since they are areas with the greatest need. It is possible that the FDA may not allow direct shipping to your personal physician, therefore it is important to choose the most convenient center for you and note that in your letter. If you are unable to travel, be sure to note that as shown in the letter.
Next, copy the following letter prepared for the FDA and fill in the blanks with the proper information, then paste it into an email and send it to cogforlife@aol.com Do not send it to the FDA - we will do that for you in one massive package. Do not send it to our attorneys as noted in the "CC" at the end of the letter - we will take care of that too. But please DO send it to your Congressman/Senator as noted in the CC.
You may add any comments you might like in the closing paragraph (prior to the "Thank you" if you have a specific personal need that might compel the FDA to respond favorably (ie for employment, education, etc)

Dear Ms. Glavin,
This letter serves as my formal request to personally import the Takahashi rubella vaccine from Japan to be used for (myself - Or) my child (children) ______ (insert first names). I understand that the FDA has not licensed this vaccine in the US, however the Takahashi rubella vaccine has been on the market in Europe and Asia since 1986 with a proven track record and poses no significant health risk. My family and I are unable to use the only US licensed rubella vaccine made by Merck due to the fact it is propagated on aborted fetal cell lines. Because of our religious and moral convictions there is no current product available for us in America. As our request complies with the FDA guidelines allowing personal import, I am petitioning you to allow this so that I may protect (myself - Or) my child (children) and others from contracting this disease. Because the cost of import shipping is substantial, I am requesting this along with several hundred other families so that we can all afford the vaccine costs. I am able (unable) to travel to (insert Region) to receive this vaccine. However if the FDA will agree, we will use our own private physician listed below. 1)Ship directly from Japan to central receiving in Pennsylvania. 2) From central receiving ship to other geographic regions in left column.
1) Receive doses centrally, ship to other vaccine centers, overnight, refrigerated with copies of package inserts included 2) Parents will schedule vaccination appointments with nearest center 3) Physicians in Centers to administer shots and follow up IMPORTANT - PRIVACY NOTICE: ALL INFORMATION PROVIDED WILL BE HELD IN STRICT CONFIDENCE. WE DO NOT PROVIDE YOUR NAME, ADDRESS, PHONE OR EMAIL TO ANYONE UNDER ANY CIRCUMSTANCES WITHOUT YOUR EXPRESS PERMISSION.
